Challenge Infections and Assays of lmmunity In Vivo. Monkeys were challenged with homologous or heterologous strains of P. knowlesi at various intervals after immunization. The standard inoculum of l0 s schizonts was given intravenously. Parasitemia was monitored by taking daily blood samples that were examined as Giemsa-stained thin smears. Animals in which the challenge infections showed a rapid rise in parasitemia were drug treated when about 15% of the erythrocytes were infected. Self-limited infections in immunized animals were drug terminated 3-4 days after the peak of parasitemia. Thereafter, monkeys were rechallenged with either strain of P. knowlesi or with P. cynornolgi. In some experiments, the spleen was removed surgically from monkeys before rechallenge. To determine the effects of the immunization of the host monkey on the sexual development of the parasite, A. balabacensis mosquitoes, natural vectors of P. knowlesi and P. cynornolgi in southeast Asia, were fed on monkeys from the 2nd day of detectable parasitemia to the day of peak parasitemia. Feeding was accomplished by applying caged mosquitoes to the shaved chest or abdomen of immobilized monkeys; mosquitoes readily fed and became engorged with blood through the screening of the cage. All feedings were done between 11 p.m. and 1 a.m. because of P. knowlesi's highly synchronous cycle of infectivity time when gametocytes are mature ; to mosquitoes 9 ; . Mosquitoes were dissected 7 days after feeding and oocysts growing on the gut were stained with 0.5% mercurochrome and counted. Immunity was indicated by a fall in oocyte number on the guts of mosquitoes fed on immunized and infected monkeys as compared to the number of oocytes observed on the guts of mosquitoes that were fed on monkeys that were infected but not immunized. In Vitro Assays oflmmunity. The presence of anti-gamete activity in the serum of immunized monkeys could be demonstrated by two in vitro tests: a ; Gametocyte-containing blood drawn from a P. knowlesi infected monkey was washed in saline, resuspended in various sera, and fed to mosquitoes through a feeding device consisting of a membrane-covered, water-jacketed 37C ; chamber 10 ; . Mosquitoes fed through the membrane on the resuspended blood much as they would feed on a monkey. Anti-gamete activity of a test serum was determined by the reduction in numbers of oocysts on the guts of mosquitoes fed on the gametocyte suspension in test serum as compared to the numbers of oocysts developed on the guts of mosquitoes fed on a suspension of similarly infected erythrocytes suspended in normal serum. b ; Blood drawn from a P. knowlesi infected monkey was washed, resuspended in test serum, and observed with a phase-contrast microscope. Microgametocytes initiate exflagellation 15-20 rain after suspension in serum at room temperature and microgametes show sperm-like activity for over 30 min. In the presence of immune serum, microgametocytes initiate exflagellation but the resultant microgametes cease movement within 2 rain. A more sensitive variant of this second test utilized a preparation of purified microgametes which were then incubated with test serum at room temperature. In the presence of anti-gamete antibodies in the seurm, in contast to what happens in normal serum, movement of microgametes ceases in 30 s. Both normal and immune serum samples used in both the in vitro tests were inactivated by heating at 56C for 30 min before use.
Akathisia has been well documented as a common and distressing side effect of antipsychotic drugs and an important cause of poor drug compliance. However, even in psychiatric settings, it is not recognised readily. In one study of movement disorders induced by neuroleptics, akathisia was diagnosed in only 26% of patients who had it.1 Akathisia is common in general medical settings, especially when patients are taking antiemetics. In cancer patients undergoing chemotherapy, 50% of patients met the diagnostic threshold of akathisia, 2 yet 75% stated they would not have reported the symptoms of akathisia. Therefore, diagnosis can easily be missed if it relies on patients' reports. Antidepressants are another group of drugs known to cause akathisia 35 but are not as well recognised. The list of drugs reported to cause akathisia has been growing box ; and the disturbance of the serotonin or dopamine system has been postulated in the aetiology of the disorder. As our cases illustrate, akathisia may manifest in various ways and is not necessarily easily recognisable as restlessness. The distress associated with the unpleasant symptoms of akathisia may lead to behavioural disturbance on the ward and to the use of and vermox and mexitil, for instance, tramadol.

In the other two cases, neither purB nor Nalr cotransduced with Camr. ; Nalr cotransduced with Camr in 10 of original isolates that showed no growth factor requirement. Thus, most of the Nalr auxotrophs were caused by transposon insertion, but many of the original Nalr isolates showing normal growth were caused by mutation other than the insertion of the transposon. The locations of the transposon insertions were verified by PCR for two icdA and two purB mutants. For each type of mutant icdA and purB ; , one insertion was within the ORF and one was immediately upstream. The cysteine-requiring mutant responded to cysteine, sulfide, or sulfite, and so it was blocked in the reduction of sulfate to sulfite 21, 30 ; . PCR showed that the transposon insertion was within the cysH gene encoding 3 -phosphoadenosine 5 phosphosulfate PAPS ; sulfotransferase. A methionine-requiring mutant isolated earlier when testing the procedure responded to vitamin B12, as well, but not to homocysteinethiolactone. This pinpointed the mutational defect to the last step in methionine synthesis, the formation of methionine by homocysteine methyltransferase, which is the product of the gene metE. PCR verified that the transposon insertion was within metE. Characterizing the role of the AcrAB-TolC and EmrABTolC efflux pumps in resistance. In order to see if the resistance of our mutants to Nal was associated with its efflux through one of these pumps, we modified the Nalr mutants so that they also contained acrAB, tolC, or emrB mutations see Materials and Methods ; . Streaking onto tryptone agar containing nalidixic acid showed that emrB: : kan had no obvious effect on the Nal resistance of icdA five tested ; , purB two tested ; , metE, or cysH mutants. However, each of the same mutants became Nal sensitive when the organism was deficient in the AcrAB-TolC pump because of defective AcrAB acrAB ; or TolC tolC: : Tn10 ; as shown for icdA derivatives by quantitative assay in Fig. 2 ; . We conclude that in each of the mutants, the AcrAB-TolC pump must be functional in order to express Nal resistance but that EmrAB-TolC plays no obvious role. Three signaling pathways for activating the AcrAB-TolC pump have been characterized see the introduction ; . In order to determine which of the three, if any, were responsible for triggering drug resistance in our mutants, we inserted mutations inactivating each signal pathway soxS3: : Tn10, rob: : kan, or marA: : kan ; separately into icdA and purB mutants. To our surprise, streak tests showed no diminution in resistance when any one signaling pathway was knocked out in these mutants. Quantitative assays supported the results from the streak tests Fig. 2 ; . These results suggested that either an unknown activation pathway was used or more than one of the known pathways triggered resistance in the mutants. In order to determine which possibility was correct, we deleted pairs of the signaling pathways from the same icdA and purB mutants to see if resistance was lost. Streaking onto Nal-containing medium showed that for each Nalr mutant, the loss of either MarA and SoxS or Rob and SoxS prevented the development of resistance. Quantitative assays with an icdA derivative show the results clearly Fig. 2 ; . Thus, it appears that each signaling pathway MarAB, Rob, and SoxRS ; has a role in the development of resistance to nalidixic acid in the mutants tested.
