A major therapeutic goal in PAOD is to increase perfusion, and preclinical models can be used to assess the potential utility of therapeutic modalities that might augment perfusion to the ischemic limb. In the use of preclinical models, the timing of the perfusion assessments is critical. It is often necessary to round off when calculating intramuscular and subcutaneous dosages. The dosage is rounded off to the nearest hundredth 0.01 ; of a mL when you are using a tuberculin syringe. When you calculate such a dosage, it is necessary to perform the mathematical calculation to the thousandths place, or the third decimal place, and round off the second decimal place according to the number that holds the thousandths place. If the number in the third decimal place, or thousandths place, is 5 or higher, you round up one number in the hundredths place to determine the dosage. For example, if you are calculating a dosage for a tuberculin syringe and your mathematical calculation gives you 0.147 mL, you would round it off to 0.15 mL because the 7 is greater than 5. To-date multiple studies have looked for single-nucleotide polymorphisms SMP ; that may identify patients who are at risk of the rare by significant adverse events which include: multiple sclerosis; optic neuritis; thrombocytopenia; onset or worsening psoriatic arthritis; congestive heart failure; and lymphoma. Recent works by Griffiths et al have identified SMP's linked to methotrexate induced liver cirrhosis. This important pharmacogenomics discovery may help us identify patients who are at risk to develop severe liver toxicity from methotrexate. Pharmacogenomics research is ongoing to discover susceptibility genes that will allow us to identify patients at risk of the severe but rare adverse events associated with biologics. Since these events occur infrequently it will likely take many years as well as many thousands of patients treated before significant numbers of adverse events occur which will allow us to do pharmacogenomics studies. One other approach may be to pre-screen patients and see if they have SMP's associated with those rare adverse events. For example: susceptibility loci have been identified for multiple sclerosis and these are contained in the HLA region on chromosome 6. Patients with susceptibility to optic neuritis also have associated SNP's in the HLA region including HLA-A24, B51, B38, B27 and B35. Risks for non-Hodgkin's lymphoma are associated with genes again linked to HLA including TNF308G. IL10-3575A is also linked to the risk of development of non-Hodgkin lymphoma. Multiple susceptibility genes have also been linked to the development of thrombocytopenia. Genes in the RUNX family of genes are linked to thrombocytopenia and may also be important in psoriasis and other inflammatory disorders. Again multiple genes have been linked to psoriatic arthritis and these include: HLACw0602, SEEK1, Card 15, TNF-alpha, IL1, RUNX1, and NFk and others.
