The purpose of this paper has been to illustrate the use of PROC FORMAT with the CNTLIN option to help in the process of data validation and "cleaning". Along the way, various methods of transposing data are also demonstrated. The example used here was a study in which medication data had been collected on a large number of nursing home residents, but because of many misspellings, abstraction of inappropriate items and the need to combine many drugs into broad categories for analysis, substantial cleaning was needed before the data could be reported or used in statistical analyses. We have found a prevalence of asthma of 24% in patients with morbid obesity. Of the 17 asthmatics, 9 reported resolution of their asthma, 5 improvement, and 3 identified no change in asthma. The three reporting no change in asthma were all mild asthmatics and were the only three asthmatics who did not have improvement in their reflux symptoms. Five of 48 patients had previous bariatric surgery, all by gastric restrictive procedures. Two had nonadjustable gastric bands, which needed earlier removal for stoma1 stenosis and excessive weight loss, respectively. The other three had failed gastroplasty with no long-term weight loss. Gastroesophageal reflux resolved after Lap-Band placement in four cases and improved in the other. There were two patients in whom the placement of the Lap-Band aggravated the reflux symptoms. Both were in group A. The first was a 33-year-old woman with a lo-year history of severe gastroesophageal reflux treated but not controlled preoperatively with 30 mg of labsoprazole twice daily. She continued to suffer reflux after band placement, and any fluid in the band aggravated her symptoms. Barium meal showed normal band placement, no evidence of prolapse, and good flow of barium through the region of the band. The Lap-Band was removed and Nissen fundoplication performed. Her symptoms subsequently resolved. The second patient was a 35-year-old woman who had severe symptoms of gastroesophageal reflux preoperatively and ulcerative esophagitis on endoscopy. A 24-hr pH study showed a DeMeester score of 36.5 N 14.75 ; initially and 15.5 post-Lap Band, and postoperative endoscopy was normal. However, she remains troubled by epigastric and mid-abdominal pains. Investigations have shown esophageal dysmotility and slow transit constipation. Cisapride therapy has given partial relief. She is continuing with the Lap-Band and is achieving satisfactory weight loss. Three patients reported no improvement in reflux symptoms. Sample analysis was done using selected reaction monitoring liquid chromatography tandem mass spectometry. With several systems for CYP2C19 activity two marker reactions, S-mephenytoin 4 -hydroxylation and R-omeprazole 5-hydroxylation, tested in either human liver microsomes or recombinant CYP2C19 ; , the five PPIs showed competitive inhibition of CYP2C19 activity with Ki of 0.4 to 1.5 M for lansoprazole, 2 to 6 M for omeprazole, 8 M for esomeprazole, 14 to 69 M for pantoprazole, and 17 to 21 for rabeprazole.
